Based on record review and interview, the licensee did not comply with the section cited above in that 2 out of 4 staff records (Staff 2 [S2] & Staff 4 [S4]) did not have a Health Screening available to review. Additonally, 1 out fo 4 staff members (Staff 3 [S3]) did not have any documents available to review which poses/posed a potential health, safety or personal rights risk to persons in care.

Facility will submit proof of the Health Screening (including tuberculosis test according to California Code of Regulations (CCR) Section 101216(g)(1)) for S2 and S4. Additionally the facility will submit proof (copies) of all copies of required documents for S3 including documents that meet the requirements of Health and Safety Code Section 1596.8662(b)(1), Health and Safety Code Section 1596.7995(a)(1), CCR Section 101216.2, and CCR Section 101216 (including CCR Sections: 101216(g), 101216(i), & 101216(l)(1)). The facility will submit these items to the Department by the POC due date.

Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlet B, with a level exceedance of 6.3 ppb, for a drinking fountain in classroom 1. This is a potential health and safety risk to persons in care.
