      Buildings and Grounds

      Based on observation, the licensee did not comply with the section cited above in that cleaning supplies (ajax, comet, lysol) were observed in an off limits office behind a baby gate and in a very high shelf in the children's bathroom, not behind a lock, which poses/posed a potential health, safety or personal rights risk to persons in care. No children were present in the areas and cleaning supplies/poisons were inaccessible, which is why a type B citation was issued instead of a type A.

      Plan of correction: Licensee immediately removed the items during the inspection and placed them in an off limits room which has a lock.

      Based on observation, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure required immunizations were maintained for all personnel which poses a potential health risk to children in care.

      Plan of correction: LPA discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agreed to provide proof of required immunizations for S1 to LPA by 07/05/2024.
