     Based on record review and Directors admission, the licensee did not comply with the section cited above in that a current disaster drill was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Facility agrees to conduct a disaster drill and submit a copy of the disaster drill log to community care licensing by 5/15/26.

     Buildings and Grounds

     Based on observation, the licensee did not comply with the section cited above in that cleaning supplies (ajax, comet, lysol) were observed in an off limits office behind a baby gate and in a very high shelf in the children's bathroom, not behind a lock, which poses/posed a potential health, safety or personal rights risk to persons in care. No children were present in the areas and cleaning supplies/poisons were inaccessible, which is why a type B citation was issued instead of a type A.

     Plan of correction: Licensee immediately removed the items during the inspection and placed them in an off limits room which has a lock.

     Correction due May 21, 2025. Standard 101238(g)(1).

     Administration of Child Day Care Licensing

     Based on record review, the licensee did not comply with the section cited above in that S2 did not have a current mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Facility agrees to have S2 complete mandated reporter (AB1207) training and submit copy to CCL by 6/20/25.

     Plan of correction: Facility agrees to conduct a disaster drill and submit picture of log to CCL by 5/30/25. LPA provided a blank disaster drill log to Licensee.

     General Provisions and Definitions

     Based on observation, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure required immunizations were maintained for all personnel which poses a potential health risk to children in care.

     Plan of correction: LPA discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agreed to provide proof of required immunizations for S1 to LPA by 07/05/2024.

     Child's Medical Assessments

     Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure Physician's Report (LIC 701) forms were completed and maintained for three out of four children which poses a potential health risk to children in care.

     Plan of correction: LPA discussed the requirement to obtain written medical assessments for children within the specified timeframe. Licensee agreed to provide copies of completed LIC 701 forms for C1, C2, and C3 to LPA by 07/05/2024.

     Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure LIC 613A forms were signed for two out of four children which poses a potential personal rights risk to children in care.

     Plan of correction: LPA discussed the requirement to maintain documentation of each child’s parent/authorized representative’s signature indicating acknowledgement of receipt of the LIC 613A form. Licensee agreed to provide copies of completed LIC 613A forms for C1 and C2 to LPA by 07/05/2024.
