Based on LPA observation, the Licensee failed to ensure that the facility remained free of smoking or vaping as required. On 03/23/2026 at approximately 10:02 a.m., LPA observed staff member S3 vaping in the attic/storage room located directly above the licensed child-care areas where children are in care. Vaping in or above the facility poses an immediate health, safety or personal rights risk to persons in care.

Recorded plan of correction
Licensee shall ensure that all staff are informed in writing of the facility’s no-smoking/no-vaping policy and the requirements of Section 101231. Licensee will submit proof of staff training, including a signed acknowledgment from all staff, to LPA via email by POC due date. Licensee agrees to ensure that smoking and vaping do not occur anywhere on the premises, including off- limits areas.
