Based on observation and record review, the licensee did not comply with the section cited above as teacher S1 did not have required background clearances associated to the facility – in violation of the 1/14/2016 waiver request. This poses a potential health, safety or personal rights risk to persons in care.
Plan of correction: Ms. Solis, the director, stated she will have staff S1 associated to the facility by the POC due date 2/27/26. LPA provided LIC9182 to transfer and associate S1 from facility number 376700942 to 376700943, per the waiver requirement. Email: SDIncidentReports@dss.ca.gov
