      Plan of correction: Director stated that bathroom renovations will be completed during the summer months while the facility is not operating (center closes for the summer beginning 6/2/2023). Director stated that children will use the restroom in Classroom #4, with adequate supervision provided, should a toileting emergency occur. Director to submit documentation regarding renovation details and plan for reporting renovation completion to the department, by 6/2/2023.
