Based on a file review for child C1 performed on 10/21/25, the facility was unable to provide the LPA with child C1's record during the initial investigation visit. The center director, Anais Heim stated that she was unable to locate the contents of C1's file and that no other records were available for review at the time. This poses a potential risk to persons in care.

Per center director Anais Heim, child C1 is no longer enrolled at the facility as of 10/16/25. As part of the plan of correction, during today's visit,Heim reviewed the LIC311A as well as CCR 101221 "Child's Records" and provided a signed statement to the LPA stating that she understands the regulations pertaining to children's records.

Based on interviews conducted and documents obtained, it was corroborated that staff S2 roughly handled child C1 as C1 attempted to sit up in their cot during nap time. This posed a health, safety and personal rights risk to the children in care.

Interim Director Brenda Bustmante stated that staff S2 is no longer employeed by the center. Bustamante stated that during the center's next all staff meeting on 11/30/23, staff will watch the Children's Personal Rights in Care video on the CCLD website https://ccld.childcarevideos.org/child-care-center-operators/childrens-personal-rights-in-child-care/ and review the written regulation during their all staff meeting. A sign in sheet with staff names and signatures to verify training will be submitted to the LPA via email by COB 11/30/23.

Based on a record review, the facility had one faucet (Site “I”) that exceeded the allowable levels of lead in the water. Site I had a reading of 6.7ppb. This is a potential health and safety risk to children in care.

LPA observed that the facility has temporarily removed the affected faucet “I” from service. The facility has replaced the faucet and will retest and submit test results to LPA at yang.yang@dss.ca.gov by 03/02/23. Center Director Meronek stated children in care are receiving drinking water from other unaffected faucets at the facility. Testing results have been posted and families have been notified of the plans for remediation.
