Director, center staff, and licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Recorded plan of correction
LPA informed director that the school will need to submit a sign in sheet to the department of the all staff meeting being held to discuss this incident. Additionally, director will submit a depiction of places staff will stand while supervising outdoors, and a copy of the map that will be distributed to staff outlining all gates that need to be locked on school grounds during operating hours.
