Based on record review, the licensee did not comply with the section cited above as 2 of 5 staff files reviewed by LPA had LIC 503 Health Screening reports and TB test results that are date more than 1 year prior to employment at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Director will obtain valid LIC 503 Health Screening forms and TB test results from staff and file in staff files. Copies will be sent to LPA via email at jialing.zhu@dss.ca.gov by 10/23/2026.

LPA observed director did not have a valid Mandated reporter training certificate. director stated that, director was ill at that time and forgot to renew the mandated reporter training.
