Based on observation, interview and record review, the licensee did not comply with the section cited above as they remodeled the home altering the "off limits" areas for the children without notifying the department which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee will need to update the facility sketch for the FCCH so that is accurately reflects "off limit"' areas, also licensee will provide the department with documentation that the remodel was approved by the city. Lastly, the licensee will contact fire department to ensure they do not need a new fire clearance due to the modifications made to the FCCH. Licensee will provide all documentation to LPA by the date noted above to cynthia.alvarez@dss.ca.gov or to 805-689-4212
