by: There was a change in director that was never reported to the department. The acting director has been present in her position since January 2026, and this was never reported to the department. This poses a potenital risk to the health an safety of children in care.

The facility representative agrees to submit a full directors packet, no later than the POC due date. This packet must include a photocopy of a college transcript, OR a Child Development program Director permit. A checklist was provided during todays inspection.

Based on observation, the licensee did not comply with the section cited above. In all but one of the preschool classrooms, chemicals were found behind cabinets that were not kept locked. Any harsh chemicals that mention "poison control" on them are considered poisonous and must be kept behind a key lock. This poses/posed a potential health, safety or personal rights risk to persons in care.

In reviewing the list of staff associated to the facility, Cheng Cheng Li was not listed, yet has been employed for some time. This poses an immediate health, safety or personal rights risk to persons in care. A $500.00 civil penalty is also being issued.

Based on Interviews and faciity's own admission via UIR, staff were unaware a child was left unattended in the playground area for at least one minute. This poses an immediate health and safety risk to the children in care.

AN IMMEDIATE CIVIL PENALTY OF $500 WILL BE ASSESSED. Facility Director understands the importance of providing care and supervision for children in care at all times. Facility director will conduct a training session with staff and submit the outline of topics discussed regarding the importance of child drop off, name to face transitions and making sure there is a sign in/out area available to parents. Documentation of training/proof of correction will need to be submitted to licensing no later than POC due date.

LPA observed children on cots sleeping however 4 children were awake with direct supervision of a 2nd teacher.
