Infant is missing individual infant sleep plan in infant's file and 15 minute infant sleep checks were not being conducted.

Licensee will also send a scanned copy of the completed individual infant sleep plan once received from family. Licensee will also begin conducting the 15 minute infant sleep checks and docuemnt.
