Based on observation, interview, and record review, the director has been away from the facility since 03/06/26 and a fully qualified director has been present, therefore the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

The licensee will designate a fully qualified director in place of the absent director. All director qualifications shall be submitted to the LPA for review. In addition, a signed LIC 308 with said person shall also be submitted to the department. All items can be submitted via email.
