Based on observation upon entering the inside preschool side of CCC, LPA observed clutter of toys, materials, furniture, supplies stacked and placed throughout the entrance which poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: By COB on POC date, Licensee agrees to submit to CCLD, proof of the entrance area of the CCC to be clutter /hazard free.

Based on observation the licensee did not comply with the section cited above in 2 identifiers where upon entrance to the outdoor activity space, LPA observed a pair of dirty child sized underpants at the front gate and Inside the center of the outdoor activity space, wooden beam approx. 3.5 feet in length and worn cardboard boxes placed as ground covering were observed. which poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: Director removed the underwear during the insepection. By COB on POC date, Licensee agrees to submit to CCLD, proof of the ground covers currently used to be removed/replaced.

Based on observation the licensee did not comply with the section cited above in 3 identifiers where LPA observed a wooden Garden box containing 3 rusted screws protruding from the corners, a Rusted pipe with 3 ends tied to a rope hanging from fence play yard fence, reachable by children and a baby gate zip tied to top of play structure to prevent use of slide, had a large hole in the center of the plastic with jagged edges, which poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: By COB on POC date, Licensee agrees to submit to CCLD, proof of the referenced items in the deficiency, to be removed and/or made inaccessible to children in care. During the inspection, Director removed hanging pipe and rope.

Based on observation, the licensee did not comply with the section cited above there were broken items, gardening tools and other debris in the playground area which poses a potential health, safety or personal rights risk to persons in care.
Plan of correction: Owner immediately removed the broken tricylcles, gardening tools, and some of the other items. Owner stated she will remove the other items and repair the basketball backboard and will email LPA pictures of corrections.
