During outdoor activity in an fenced yard, 3 unsupervised child wandered away from the facility for an undetermined amount of time, which poses an immediate risk to the Health, Safety or Personal Rights to children in care.

Recorded plan of correction
The licensee has added 2 separate safety devices to the gate in the front yard. She also added a child gate to the side yard to make the front yard inaccessible to children in care. Licensee met with staff to review supervision of children and inplemented name to face checks every 15 minutes and during transitions. LPA observed the changes during inspection and the POC is cleared as of 10/15/2026. This is a zero tolerance violation. An immediate $500 is assessed today and $100 per day will be assessed until corrected. Subsequent zero tolerance violations are $1000 immediate civil penalty and $100 per day will be assessed until corrected.
