Based on record review and interview, the Licensee did not comply with the above by leaving a child without supervision in a classroom. This posed an immediate risk to the health or safety of children in care.

The facility has initiated corrective actions and submitted plans to the Regional Office. Two staff members have been terminated as well. The facility will send LPA all documentation regarding the emergency evacuation procedures and signed meeting agendas. All staff who attend shall sign and print their names. Also, LPAs informed Site Director, Nicole Porter, to provide a copy of this licensing report dated 2/10/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report.
A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
This report must be available in the facility for public review. Notice of site visit shall be posted for 30 days from today's visit. Appeal rights were provided and explained.
