Based on record review and interview, the licensee did not ensure the child's court ordered schedule was adhered to, which poses a potential health, safety, or personal rights risk to persons in care.

The facility has created a pick-up schedule calendar for the child's teachers that shows the dates when authorized representatives may pick them up. The facility has discussed with the child's parents about what is required for a change in pick-up times. Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. Once received, the facility took immediate and proper actions to remediate the fixtures. This poses a potential health, safety, or personal rights risk to persons in care.

Children were sent out to play without supervision from teachers. Teachers did not communicate properly regarding who was available to supervise children which poses a potential health and safety risk to children in care.
