Based on interview and record review, the licensee did not comply with the section cited above in that on December 2, 2025, there was an absence of supervision resulting in C1 being left unsupervised in the restroom which poses an immediate, safety, or personal rights risk to persons in care.

Facility Representative Chavez stated that she will conduct a training with staff regarding supervision and transitioning.
