      Teacher-Child Ratio

      Based on observation and interview, the licensee did not comply with the section cited above. Upon arrival, LPA observed teacher Olivia Casarrubias-Quinn caring for 15 children without any other adults present which poses an immediate health, safety or personal rights risk to persons in care.

      Plan of correction: Community Liaison staff #3 (S3) arrived at 8:22 a.m for support to bring the facility back into ratio. LPA observed assistant staff #2 (S2) arrive at 8:25 a.m.

      Correction due Mar 1, 2024. Standard 101216.3(a).

      Sign In and Sign Out

      Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file reviews, LPA observed only 12 out of the 15 children present were signed in which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Teacher stated she will send an email to all parents of the importance of signing children in on the sign in/sign out sheet. Teacher stated she would provide a copy of the email by POC due date.

      Correction due Mar 8, 2024. Standard 101229.1(a)(1).
