Indoor Activity Space
Based on observation of the preschool classrooms, the floors, rugs, and bathroom walls were observed to be dirty and need a deep clean. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Plan of correction: Facility will deep clean all floors and walls, including bathrooms, in facility so that they are free of dirt and grime. Faclity will also either wash, deep clean, or replace all of the rugs in the preschool classrooms. All cleaning will be completed by POC due date Site Supervisor will inform LPA Pacheco when everything is clean. LPA will return to facility once it is clean to ensure corrections have been made.

Based on record review, the last time a fire drill was completed was 12/16/2022. Based on this information, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Plan of correction: Licensee will provide documentation stating how he will insure that moving forward staff will conduct a fire drill every six months to comply with the regulation above. Licensee will also complete a fire drill by the plan of correction due date and provide documentation. Documentation is to be provided by, 12/12/2023, via email to LPA Erica Pacheco.

Responsibility for Providing Care and Supervision
Based on LPAs observation. In Classroom P3 LPAs observed a child using a bathroom stall adjacent to the classroom unattended. LPAs observed one staff sitting at a table working with children and a second staff with her back turned to the bathroom organizing classroom materials. This poses an immediate health, safety or personal rights risk to children in care.
Plan of correction: Licensee agreed to watch the video Supervising Children in Child Care Centers on ccld.childcarevideos.org with staff and provide training regarding care and supervision and submit a written plan of who attended the training and how this regulation will be complied with going forward.
