Based on observation, the indoor play area near the outside play area had several white, mineral fiber ceiling planks that are broken, the ceiling fan is broken, the walls are unclean, there is smoke detector hanging from the ceiling, a broken fire alarm with exposed wiring on the wall near the carbon monoxide detector. In the school-age room, there is a broken arcade machine and a bathroom stall that is not open for use. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Facility will repair all broken ceiling planks, the broken ceiling fan, the smoke detector that is hanging, and the missing thermostat on the wall with exposed wiring. The facility will remove or repair the broken arcade machine and reopen the bathroom stall that is not currently open for use. Director or Site Supervisor will contact LPA when all repairs are complete by the POC due date. LPA will return to the facility to observe the repairs in person for a plan of correction visit.
