During inspection, LPA observed two small blue couches in room P3 that have several stains on them. LPA also observed two small brown couches in the Pre-K room where the upholstry is ripping on the arm rests. This prevents the furniture from being sanitary. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Plan of correction: The facility is to have furniture replaced, repaired or removed. Facility will send photos showing the plan of correction to LPA via email by close of business 03/12/2024.
