by interview and observation of video footage. Information obtained indicated that child #1 was left in Room 4 unsupervised for approximately 30 seconds. This poses an immediate risk to the health, safety or personal rights of children in care. A civil penalty of $500 was assessed.

Site Supervisor provided documentation that a training on active supervision, staff responsibilities, and updated procedures for keeping account of children present, along with staff attendance sign in sheet, was completed at the facility. Site Supervisor also removed the blind spot in Room 4 and has provided training materials for a future training to be conducted with all staff on 10/12/2026. Deficiency cleared. SHAW AVE, FRESNO, CA 93710 FACILITY NUMBER: 203801367 VISIT DATE: 09/18/2026 Trainings were done with staff regarding how to position staff to prevent an incident from occurring. LPA advised Director that an injury to the face is considered a head injury and should be reported immediately to parents or authorized representatives, as stated in the parent handbook. Based on the information received, the facility did not take appropriate measures to address this incident, which includes following proper policies, procedures, or reporting requirements. This poses a potential risk to the health, safety, or personal rights of children in care. Exit interview conducted and report was reviewed with Director, Marcela Esparza. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, there is deficiency being cited during today’s inspection. (See next page, LIC 809-D).
