Staff #1 (S1) grabbed C1 and forced C1 down to the ground, crossed C1’s arms across C1’s body. S1 restrained C1. This poses an immediate risk to the health, safety and personal rights of children in care.

The Director is to provided the Department with a written plan, by POC due date, that will provide the training materials along with specific courses that the Director will take regarding de-escalation and re-direction of children. During tour of facility at 3:25pm it was observed in classroom 2 that facility did not ensure that cleaning products were locked and made inaccesible to children. This posed an immediate risk to children in care

Based off of interview of staff and management it was stated that staff did engage in a verbal altercation 3/31/2023 while children and parents were present. This poses a potential risk to children in care.
