Based on observation, record review, and interviews, the facility failed to comply with the license issued for age ranges 2 through entry into 1st grade by enrolling and accepting Child 1 prior to the child's second birthday, which poses an immediate health, safety, or personal rights risk to children in care.

Licensee and Staff 1 shall immediately discontinue enrolling and accepting children into the program prior to reaching their 2nd birthday. Any children currently in care shall be disenrolled and re-enrolled after reaching their 2nd birthday. Licensee agrees to add an addendum to the admissions agreement that clearly states no child shall be present in the Center until after the child has reached their 2nd birthday. Parents/Licensee will sign and date the addendum and shall submit to LPA via postal mail or email by 7/22/2022.
