LPA observed a large wooden multi-level play structure in Outdoor Play Area 1 to be completely surrounded by caution tape. In addition, LPAs observed holes in cushioning material below structures and paint chipping from all wooden play structures in outdoor play area. This poses a potential health, safety, and personal rights risk to children in care.
