Plan of correction: Licensee will esnure the outlet covers are fixed to ensure the safety of the children. Licensee will send proof of corrections to LPA by POC date of 11/14/2025

Plan of correction: Per director, saftey latches will be placed on the bottom cabinet of the sink and the cleaning solutions will be moved from the lazy suzy to the bottom cabinet of the sink making cleaning solutions inaccessible.
