      Based on observation,interview and record review, the licensee did not comply with the section cited above in that staff # 1 and staff # 2 did not have current mandated reporter certificat which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Per faclity representative staff wil take training and submit certificate to LPA by POC due date of 4/10/2026.

      Based on observation, the licensee did not comply with the section cited above in LPAs observed trash can in the play yard with food inside but no lid on the trash; LPAs also observed the trash can located int he cafeteria with no lid which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Lead teacher will ensure that the trash cans used for soild waste have a tghtfitted lid. Lead teacher will provide proof to LPA by POC date (09/29/2023).
