      General Provisions and Definitions

      Based on observation and record review, 3 staff are missing MMR records in staff files, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Staff will provide the Director of the missing immunization records by 06/16/25. Director will scan the documents to LPA Chambers email byb 06/16/25.

      Correction due Jun 16, 2025. Standard 1596.7995(a)(1).

      Based on observation and record review, one staff file is missing a TB clearance record in the staff file, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Staff will provide the director with the missing TB clearance record and Director will place in the file. Director will scan the document to LPA by 06/16/2025 via email.
