Infant Safe Sleep

Based on observation, interview, and record reviews, the licensee did not comply with the section cited above in 2 count out of 2 count objects, persons missing 15 minute infant sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA provided a copies of 15 minute infant sleep check charts. Licensee will begin documenting starting immediately, 11/20/2025 and will provide LPA picture proof that 15 minutes infant sleep checks are being conducted daily for each infant in care.
Licensee failed to provide safe toys and play equipment when she failed to follow the manufacturer’s directions of the "My 1st Jump n Slide" which revealed 1) the age range 3 years - 8 years and 2) the product must be used with anchors provided and installed properly. Child #1 was not yet 3 years old and the bounce house was not anchored. This poses a potential Health, Safety or Personal Rights risk to children in care.
