Supervision shall include visual observation. This requirement was not met as evidenced by a child in care having two injuries resulting in medical treatment. Neither incident was not observed by any staff present during the incidents.

Director has created a visual zone chart for staff to enter initials to cover all zones of the playground. Director will meet with staff either individually or as a group to discuss active supervision and have all staff sign a document stating they understand what constant visual supervision looks like and will attest to follow all regulations regarding constant supervision. Send copies of LIC 9224 to LPA and other documents to LPA at tammy.dutra@dss.ca.gov. Director agrees to send a plan to LPA by 3/4/26.
