Based on record review, the licensee did not comply with the section cited above in that ten children files were missing the Acknowledgement of Receipt of Licensing Reports (LIC 9224) from the type A citation issued on 08/30/2021. This poses a potential health, safety or personal rights risk to persons in care.

Ms. Merk stated she shall provide a copy of the report from 08/30/2021 to parents and have them complete the LIC 9224 form. In addition, all other files shall be reviewed to ensure the LIC 9224 form is on file for all other kids in care. Copies of the LIC 9224 shall be sent the Fresno Regional Office by the given due date.
