Licensee stated a work order was placed for a new toilet lid on January 6, 2026. Licensee made the toilet inaccessible to children by covering the opening to the toilet. Licensee stated there is one additional toilet for the children to use at the facility. Licensee stated she will send a photo of the new toilet lid to the department no later than 2/6/2026.
