  Plan of correction: Per Licensee they will send proof to Licensing by POC date.

  Lead Testing
  Based on interview, the licensee did not comply with the section cited above. During today's visit, the facility does not complete the Water Lead Testing. According to the owner, she will submit the requrest to the water testing agency to schedule an appointment. which poses/posed a potential health, safety or personal rights risk to persons in care.
  Plan of correction: The owner will submit the request for the water testing agency and let LPA know the appointment.

  General Provisions and Definitions
  Based on record review, the licensee did not comply with the section cited above. LPA observed 4 teachers and 3 of teachers do not have MMR or Dtap,which poses/posed a potential health, safety or personal rights risk to persons in care.
  Plan of correction: The owner will check and make sure all the staff have their MMR or Dtap by 12/20/24

  Based on record review, the licensee did not comply with the section cited above. During today's visit, LPA reviewed 5 children's files. 3 children do not have physical exam or TB risk assessment, which poses/posed a potential health, safety or personal rights risk to persons in care.
  Plan of correction: The owner will make sure all the children have physical exam and TB test in the file. The owner will email LPA to confirm all files have Physical exam and TB risk assessment.
