Based on interviews, on 11/2/2022, due to lack of communication and/or training, staff did not timely change a child's diaper prior to pick up from the facility, resulting in leakage and wet clothing. This is a potential risk to the health and safety of children in care.

Based on LPA's interview with the director and observation of 51 children indoors who were not wearing face coverings and Licensee not enforcing the use of face masks/coverings of children. This is an immediate hazard to the health of children due to the COVID-19 pandemic which is currently on the rise.

Director will also make disposable masks available to the children upon entering the classroom if needed.
