Based on interview with director and LPA Flores observation on 8/15/23 the facility playground had spiderwebs in five playhouses that were stationed next to the fence. This poses a potential risk to the health, safety or personal rights to children in care.

Director will provide LPA Flores a plan of correction with the staff's signatures and will send LPA Flores photos of the spider webs removed and aras cleaned via email.
