Staff will complete sleep logs for C3, C6-C8 for one month and will send completed logs to LPA by COB 5/14/26. Licensee will ensure the logs include the children's name, DOB, the complete date of the check, the time of the check, and any signs of distress if applicable. Licensee will also ensure the staff member who completed the 15-minute sleep check initials the documented check.
