      101161 Limitations on Capacity

      Based on observation and interview, the licensee did not comply with the section cited above C6 is 2 years of age and was moved to preschool classroom without approval which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: On 04/14/2026 C6 was moved into the preschool program although they were enrolled into the toddler option component. Licensee has moved C6 back into the toddler option classroom until the exception is approved. Licensee also stated they plan on removing the toddler option from their infant program license. While LPA's were present C6 was moved back into the proper classroom until C6 is approved.

      Based on observation, the licensee did not comply with the section cited above in LPA witnessed play sets with spider webbs in multiple areas, which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director will submit proof of training with staff to ensure cleaning is conducted accurately and daily to sweep off spider webbs. Documentation will include sign in sheets and topics covered.

      Based on observation and interview, the licensee did not comply with the section cited above in outside shed was unlocked with chemicals, old paint cans, that could cause harm to children in care if touched, This poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Licensee will have the shed locked at all times making the area inaccessible to children in care by 05/15/24. Licensee will send LPA Flores via email photo of the shed being locked, and inaccessible to children in care by 5/15/24.
