A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions with the exception of Fawad Zahoor Alvi which poses an immediate health, safety or personal rights risk to persons in care..

During children file reviews at 9:50 AM, LPA observed Child (C1) did not have form LIC 9227 Individual Infant Sleeping Plan inside file. Licensee stated she was unaware of the form which poses/posed a potential health, safety or personal rights risk to persons in care.
