On 04/05/2022 at 4:27 PM, child #1 was left unsupervised behind the enclosed play yard gate for an undetermined amount of time until staff noticed the gate door was left opened and the latch broken. The staff went to retrieve child #1 behind the gate. This poses an immediate health and safety risk to children in care.

The Director stated they repaired the play yard gate latch and conducted an in-service to staff on supervision due to the incident with child #1. The Director will provide proof of an in-service to staff on care and supervision and will submit proof of in-service by 05/11/2022. The Director will also submit a written plan on how the facility will ensure that this type of incident does not occur again and will submit the written plan along with the in-service to staff to the Licensing Office by 05/11/2022.
