Director stated S1 is no longer employed with the facility. Director stated training on the proper handling of children was provided to all Infant Center staff and she will provide LPA with a written description of the training and staff sign-in sheets by 01/12/2026.

Based on interview and record review, the licensee did not comply with the section cited above, as Child 1 (C1) exited the classroom alone and was found on the playground without supervision, which poses an immediate health, safety, and personal rights risk to persons in care.

Director stated that she has retrained staff on supervision. Director stated that staff will view the video Supervising Children in Child Care Centers on CCLD website and submit a staff sign in sheet, also a written plan of correction for supervision procedure to the SDRO by 9/8/2023.
