Based on observation, interviews, At 10:00am, Director spoke with project coordinator to obtain a copy of the lead sampling test, project coordinator informed director that the church had not conducted a lead sampling test at the facility. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Director stated they will begin the process to call and schedule an appointment for a water testing company to come out and get the lead sampling testing done. Director will inform the licensing department of the scheduled date for the lead sampling test and provide a copy of the lead sampling test to licensing office by the due date. Director had project coordinator turn off the water for drinking fountains and children will only have their water bottles refilled from filtered dispensor.
