MinorBased on the sign in sheets provided combined RM 105 and RM 104 had total 20 children in attendance for 08/31/26 with one qualified teacher and two aides. (one with 6 units and with no units.) Additionally, teacher was left alone with more than 12 children in RM 105 when diaper change was conducted in the RM 104.

Per Director, facility will have assigned staff schedule for each room. Meeting will be conducted with staff to review ratio requirements and units requirements. Staff will log out and log in for each room that they assist. Agenda will be send to LPA by POC date.
