Based on observations,the licensee did not comply with the section cited above. During the tour, LPA observed lower outdoor area`s wooden flooring was missing piecies and one plant in the upper outdoor play area per Google showed toxic,which poses/posed a potential health, safety or personal rights risk to persons in care.
