      Buildings and Grounds

      Based on observation, the director did not comply with the section cited above as director did not ensure poisons, including an OdoBan disinfectant bottle, were locked which poses a potential safety risk to children in care. The poisons were located in a lower cabinet that was not secured.

      Plan of correction: The day care children were in another classroom at the time of inspection. LPA discussed the requirement to ensure storage areas for poisons are locked with a key or combination lock. Director agreed to install a lock for the storage areas of the poisons and provide proof to LPA by 07/30/2025.

      Based on observation and interview conducted, the director did not comply with the section cited above as director did not ensure a functioning carbon monoxide detector was present which poses a potential safety risk to children in care.

      Plan of correction: LPA discussed the requirement to ensure there is one or more functioning carbon monoxide detectors in the facility. Director agreed to provide proof of a functioning carbon monoxide detector to LPA by 07/30/2025.

      Buildings and Grounds

      Based on observation, the licensee did not comply with the section cited above, at t 10:41 AM, LPA observed a child sized water fountain, base made of PVC pipe with nails protruding and filled with leaves and debris, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: The licensee agrees to clean and repair the water fountain and provide photos of the repair on or before 01/22/2024. The licensee stated they will make the water fountain inaccessible to children and use water bottles during outdoor play until the repair is complete.

      General Provisions and Definitions

      Based on observaton and record review, the licensee did not comply with the section cited above where 2 preschool staff present and working with children did not have proof of immunization records for the measles, pertussis, influenza vaccinations or letter to decline the influenza vaccination which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: The licensee stated they will provide proof of immunization records on or before close of business day 01/18/2024, by fax or e-mail.
