Based on observations and interviews Director did not report the incident in a timely manner to the licensing office. This poses a potential risk to the health and safety to the children in care.

Director stated they wanted to conduct their internal investigation and get the green light from headquarters before reporting the incident to licensing. Director reported the incident via phone call and sent in the LIC624 on 1/11/23. Director will make sure they report the incident in a timely manner.

Based on lab report analysis water from faucet E (10.0 parts per billion). These faucets samples taken on 05/06/2022 indicate levels of lead in exceedance.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
Facility staff followed Written Directives in PIN 21-21 CCP for lead water testing. Licensing Forms completed; 9275, 9276 and facility sketch submitted. Notification sent out to parents, water outlet flushed (not in use), conditioning for 3 weeks, water filter installed onto pipping, Reusable pitchers of water from the other PASS sinks readily available for the staff and children. Director stated that she will submit new restest results to LPA via email to dianna.valdezsantana@dss.ca.gov. In addition facility provides bottled water for children to drink.
