Facility conducted an internal investigation, resulting in termination of S1’s employment as of 1/13/26. The whole staff will review and provide a statement of understanding Personal Rights CCR section 101223, to the LPA via email by due date. by: S2 did not verify if C1’s authorized representative was there to pick up C1. Staff #2 (S2) did not C1 walked out of the facility, alone, and was unsupervised for approx. 10 minutes until C1 reached C1’s home. This poses an immediate health safety, and personal rights risk to the children in care. Immediate Civil Penalty was assessed.
