      Based on observation and record review, the licensee did not comply with the section cited above in 4 of 6 staff are missing immunizatin records which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Administration will ensure that all staff files are updated to show current immunizations. Admin will email current immunizations to LPA at ranita.richmond@dss.ca.gov by 02/20/26.
