      Based on observation and statement from director staff #1 has been present at the facility since 09/13/2021 the director did not comply with the section cited above in 1 out of 7 which poses an immediate health, safety or personal rights risk to persons in care.

      Plan of correction: Staff #1 vacated the facility during today's inspection and will not return until cleared in guardian.

      Correction due Nov 3, 2021. Standard 101170(e)(1).
