Based on interviews, 4 out of 6 staff members stated they witnessed staff interrupting the daycare children while napping. Staff #3 (S3) stated they observed S1 moving a crib from one room to the other. S3 stated, children should be sleeping without being interrupted and moving their crib is interrupting their sleep, which poses a potential health and safety risk to the children in care.

LPA interviews with Reporting Party, Director, 4 staff members and 1 parent, it was discovered that subject child and other children who attend the facility are receiving a diaper change over 2.5 hours after their last diaper change, causing a rash on the child. Children need to be changed as needed. This poses an immediate risk to the personal rights of children in care.
