EL SEGUNDO, CA 90245 FACILITY NUMBER: 197494464 VISIT DATE: 07/08/2026 All of the children returned to school as directed in the physicians notes.
It was reported that staff are cleaning the classrooms at least twice daily.
Based on the information obtained through LPAs review of the children's files it has been determined that although parents and authorized representatives were notified in a timely manner, by June 18,2026 there were 4 confirmed cases of hand foot and mouth disease , and the incident was not reported to the department until June 24,2026. Therefore the Incident was not reported as indicated in Title 22 regulations 101212 (d)(1) (E) for reporting Unusual Incidents. One (1) Type B Citation is being cited today for Reporting Requirement.
An exit interview was conducted and a copy of this report and Appeal Rights were provided to Acting Director Yolaina Urbina.
